Attention Parents of Fifth Graders and Eighth Grade Students
Beginning Fall of 2012, students entering sixth and ninth grades will
be required to provide proof of Tdap vaccination. Tdap is a vaccine
licensed and recommended to protect pre-teens, adolescents and adults
against tetanus, diphtheria and pertussis (whooping cough).
Students in these grades without one of the following will be subject to exclusion from school:
1. Proof of Tdap vaccination
2. An approved medical or religious exemption on file with the school
3. An appointment to receive the Tdap shot during the school year
The school physicals required for entrance into 6th and 9th grade
must also be on the revised Department of Human Services (DHS) form.
Please make sure that your student receives this immunization along with
the school physical so that they will be ready to start school on
Wednesday August 22, 2012.
