Attention all 5th grade students/families:
Illinois state law requires that students entering 6th grade must submit evidence of a physical examination. The physical examination must be dated within 1 year prior to the first day of school.
Beginning next school year (Fall of 2013) all students entering into 6th grade will also be required to show proof of receipt of one dose of Tdap vaccine (combined tetanus, diphtheria, pertussis) regardless of the interval since the last DTap, DT or Td dose. Your child should receive a Tdap with their required 6th grade school physical if necessary.
Per CUSD 200 policy, students who do not have a current physical on file by the first day of school will not be allowed to participate in PE until a physical is received. Students will be excluded from school several weeks after the start of school if physicals and immunizations are not current.
Students entering 6th grade in the fall must also submit evidence of a dental examination by May 15 of the 2013-2014 school year. Dental exams must be completed within 18 months prior to May 15, 2014.
All health forms are available for download on the district website or in the Health Office
Purging Student Files
Portions of the temporary student records for students moving on to (middle school or high school) will be updated and/or deleted prior to the end of the school year. If you would like to come in to inspect or copy any records, please call the school office. All immunization records, accident reports, academic transcripts (grades, class rank, graduation date, grade level achieved and scores on college entrance exams), records of release of information contained in the permanent record, scores on the Prairie State Achievement Test, honors and awards received and information about a student’s extracurricular activities will be kept as part of the permanent school records.
