    PUNS survey – Prioritization of Urgency of Need for Services.
    Registering with the Prioritization of Urgency of Need for Services (PUNS) database is the first and most important step you and your family can take to receive assistance.

    What is the PUNS Form?

    The “Illinois Prioritization of Urgency of Need for Services” or PUNS is a data collection process that will help the Illinois Department of Human Services, Division of Developmental Disabilities determine the true needs of persons with developmental disabilities so that the disability support system can be developed to meet those needs now and in the future.

    The PUNS process (Illinois Prioritization of Urgency of Need for Services) will allow Illinois to establish a database of developmentally disabled individuals and also prioritize their need for services. The form is to be completed with a face to face conversation with the individual to be served, and the individual’s guardian, primary care giver, or parent and the individual’s ISC agency.
