Welcome to the Jefferson Health Office. Our office is staffed with a Nurse at all times to provide care for our students with any acute or chronic medical needs during the school day. School Physical and Immunizations: A school physical (The Child Health Examination Form), completed and signed by your child’s primary medical provider, and updated immunizations are required for children entering Pre-K, Kindergarten, 6th grade, 9th grade and for all NEW students entering school for the first time. Physicals must be completed no earlier than one year prior to the student’s first day of school. Immunizations include Varicella (chicken pox), Hepatitis B series, MMR, Polio series, DTaP series, Hepatitis A, Hib series, and PCV series. All health requirements are due by the first school day after Labor Day to avoid school exclusion. Students who enroll after the first day of school have a thirty (30) day grace period to provide evidence of a physical examination and immunizations requirements. Students who do not meet the health requirements will be excluded and will not be permitted to attend school per District Policy 7:100 Health,Eye and Dental Examinations; Immunizations and Exclusion of Students. Unless an exemption or extension applies, the failure to comply with the health requirements by the districtwide deadline will result in the student's exemption from school until the required health forms are presented to the District. Medications: In order for students to receive any medication at school, whether it be prescription or over-the-counter, the district medication administration form must be completed and signed by the provider and parent/guardian. This form has to be renewed each school year.

Chronic Medical Conditions: Should your child have a health condition such as Asthma, Allergies, Seizure Disorder, or Diabetes, please have an emergency action plan completed and signed by their provider and submitted to the school nurse. It is of utmost importance that any health or medical concerns your child may have are communicated with your child’s school nurse.

Communicable Illnesses: To help prevent and control the spread of illnesses and to help keep our students and community healthy, it is recommended to keep your child home if they are ill. Should your child test positive for a communicable illness such as but not limited to Influenza, RSV, Strep Throat, Hand Foot Mouth, Whooping Cough, etc.; it is helpful when that information is included when calling your child in for the day. For more information on when to keep your child home from school, please see below where it reads “Too Sick for School.”
Dental: Dental examinations are required for Kindergarten and 2nd grade students only. Examinations should be completed within 18 months of May 15th of the current school year.

Vision: Eye Examinations are required for students entering Kindergarten and students entering an Illinois school for the first time. The Eye Examination Report is due by October 15th of the child’s Kindergarten school year.
