Home Hospital Instruction

Consistent with the ISBE guidance, Section 226.300 Continuum of Alternative Placement Options, Section 1.520 Home and Hospital Instruction, and District 200 Home/Hospital policy, students are eligible for home/hospital instruction if:

a. The child receives virtual services at home or in a hospital because he or she is unable to attend school elsewhere due to a medical condition. A Director will be consulted for any request for in-person services within a public setting.

b. When a student eligible for special education services has a medical condition that will cause an absence for two or more consecutive weeks of school or ongoing intermittent absences*,the IEP Team for that child shall meet to determine the type and amount of home or hospital services. Such consideration shall be based upon a written statement from a physician licensed to practice medicine in all its branches, a licensed physician assistant, or a licensed advanced practice registered nurse, which specifies:

1. The child’s medical condition;
2. The impact on the child’s ability to participate in education (the child’s physical and mental level of tolerance for receiving educational services); and
3. The anticipated duration or nature of the child’s absence from school.

"Ongoing intermittent basis” means that the child’s medical condition is of such a nature or severity that it is anticipated that the child will be absent from school due to the medical condition for periods of at least 2 days at a time multiple times during the school year totaling at least 10 days or more of absences. There shall be no requirement that a child be absent from school a minimum number of days before the child qualifies for home or hospital instruction. (Section 14-13.01(a) of the School Code [105 ILCS 5/14-13.01(a)].)

To initiate the process for requesting Home Instruction, parents must submit a written request by completing the CUSD 200 Medical Certification Form (“Med Cert”) and returning it to the respective School Nurse.
