Transition to Jefferson from Early Intervention
Welcome to Community Unit School District 200 Early Childhood! If you have accessed this page, you may have a child who will be transitioning to our Early Childhood program from Early Intervention (Child and Family Connections). These pages are meant to help you navigate and understand the system that we have in place to help you and your family, here at Jefferson Early Childhood Center.
Transition Meeting
Approximately 6 months before your child's birthday, Dayone Pact will be contacting you to sign a release of information form to have the ability to refer your child to Jefferson Early Childhood Center. Once the permission is signed, records regarding your child's evaluation, goals and progress will be sent to Jefferson so we can begin the process of understanding the educational needs of your child.
After we receive the information you will be invited to a Transition meeting here at Jefferson Early Childhood Center or via telephone conference. At the Transition Meeting you will meet a representative of Jefferson Early Childhood Center and a representative from Dayone Pact. At that time, we will be reviewing rules and regulations involving special education evaluations, services and placement options. We will also be discussing eligibility criteria for receiving these services. District policy states that you will need to provide your child’s original birth certificate and three documents to prove residency. These forms of identification will be copied and placed in your child’s file. Click here to see the documents necessary for proof of residency.
At this meeting you will also be given dates for an educational evaluation of your child and for the meeting to discuss eligibility for services. We look forward to meeting you and your child and assisting in the transition from Early Intervention to the Early Childhood services we offer here at Jefferson Early Childhood Center.
Review of Existing Data Meeting & Evaluation
The evaluation process starts with a Review of Existing Data Meeting. At this meeting the Evaluation team will determine the areas in which we need to gather additional information. With your consent, the evaluation will follow this plan when evaluating your child. Our evaluations are conducted in a play-based manner with parent interviews conducted concurrently. This meeting will take approximately 1-2 hrs and is dependent on the areas which need to be evaluated.
Initial Evaluation Meeting
The results of the evaluation will be discussed during the last meeting prior to your child's third birthday. At that meeting, the team (parents and school staff) will determine your child’s eligibility for special education services based on state and federal guidelines. If your child does qualify for services, an Individualized Educational Program (IEP) will be developed.
