Greetings Transition Student Families,
I am the Social Worker for the Transition Program. I look forward to working with you and your young adult. For the last twelve years, I have been striving to work with students, families, staff, and community resources for future success. I enjoy and am available to discuss any pertinent information you would like to share with me about your student in regards to strengths, goals, and future concerns for the time that your student is enrolled in the Transition Program to promote higher social and emotional wellbeing.
Throughout the year, I facilitate individual and group social skill activities within your student’s schedules. These social-emotional learning skill groups focus on adult expectations, self-advocacy, self-determination, team-building, transition/college success, goal development, communicating feelings, demonstrating appropriate job-related social skills, successful adult behavior and life skills, friendship and relationship skills, problem solving techniques, participating in designated social freetime/games, planning weekend social events, and engaging in social and fitness community outings. I also co-facilitate ongoing activities geared towards promoting healthy adult nutrition and exercise, personal safety, healthy relationship skills, boundaries, and a review of adult human sexuality.
Most importantly, I have designated time within the week for families to consult regarding creating a student-centered future plan for post-transition. These meetings can be helpful in making sure that together we explore and apply for all possible government funding resources (PAS agencies or DRS), social security benefits (SSI/SSDI), public aid (Medicaid), guardianship, referrals to create a special needs trust or ABLE accounts, navigating transportation resources, and developing a structured plan for when your young adult exits the public school system. I also support families with linkages to post-secondary education programs, adult day programs, or local community agencies.
