The Illinois school code requires that students entering school for the first time, (early childhood, kindergarten, or first grade), students entering kindergarten, sixth and ninth grades, and transfer students must submit evidence of a physical examination and a complete record of immunizations.
The completed physical examination and record of immunization form is expected at the school health office upon enrollment/ registration and required by the first day of student attendance.
Appointment dates for obtaining a physical exam do not meet the school requirement.
Physical examinations must be done by a licensed physician, advanced practice nurse, or an examining physician assistant.
Physical examinations must be dated within twelve months prior to the first day of school.
Only the State of Illinois Department of Human Services Certificate of Child Health Examination form will be accepted.
Medical objections to any physical examination or immunization must be submitted in writing by a physician and must contain the required information.
Religious objections to any physical examination or immunization must be submitted by a parent/guardian in writing and must contain the required information. Click here for the religious objection form.
Students who do not meet these requirements will be excluded and will not be permitted to attend school per District Policy:
7:100 Health, Eye, and Dental Examinations; Immunizations and Exclusion of Students - Unless an exemption or extension applies, the failure to comply with the above requirements by the first school day after Labor Day of the current school year will result in the student's exemption from school until the required health forms are presented to the District. New students who register after the first school day after Labor Day of the current school year shall have 30 days following registration to comply with the health examination and immunization regulations. If a medical reason prevents a student from receiving a required immunization by the first school day after Labor Day, the student must present, by the first school day after Labor Day, an immunization schedule and a statement of the medical reasons causing the delay. The schedule and statement of medical reasons must be signed by the physician, advanced practice registered nurse, physician assistant, or local health department responsible for administering the immunizations.
A student transferring from out-of-state who does not have the required proof of immunizations by the first school day after labor day may attend classes only if he or she has proof that an appointment for the required vaccinations is scheduled with a party authorized to submit proof of the required vaccinations. If the required proof of vaccination is not submitted within 30 days after the student is permitted to attend classes, the student may no longer attend classes until proof of the vaccinations is properly submitted.
Immunizations are required for school attendance.
Dental examinations are required for kindergarten and second grade students. The dental
exam must be conducted within one year of May 15 of the current school year.

Eye examinations are required for students entering school (in Illinois) for the first time (Kindergarten or out-of state transfer)
Medications: In order for students to receive any medication at school, you and your doctor must complete the district’s medication order form: Forms for Medication, Allergies or Asthma
Asthma: If your student has asthma, having an Asthma Action Plan at school means that we will have an awareness of what your child's triggers are and what their "green, yellow, and red" Peak Flow Zones are. Food/Peanut/Tree Nut Allergies: Please notify the health office if your child has any type of severe food allergy that may require emergency attention. Your child's medical provider will also need to agree to have this emergency medication available for your child to have at school if needed. Vision & Hearing Screenings are conducted by the DuPage County Health Department. All special education students, kindergartners, second graders, and students new to the district will be screened for vision and hearing. First and third graders will be screened for hearing. District #200 does an extra vision screening for 4th grade students. If you have any questions or concerns regarding your child’s vision or hearing, please contact us and we can screen your child for vision or hearing.
