Health Forms - During 2026 Summer forms can be brought in to Longfellow Elementary for Franklin Middle School Students. Franklin Middle School will be under construction and not open. Call for an appointment at 630.682.2060

New Registers or if you have moved but still live in our boundaries- Proof of residency forms and birth certificates MUST be brought in to our registrar, Dipti Desai, to have copies made. Please call to make an appointment, or email Dipti at
