Our Health Office is staffed all day, every day in the event that your child feels ill or has an emergency at school. Students Leaving Sick from the Nurse's Office :

For safety reasons, students are not allowed to call or text parents to pick them up without being seen by the Nurse first.

Health forms:

Below you will find some of the health forms that your student might need in order to receive the help he or she requires during the school day. We would be happy to send any hard copies of these forms home with your student, however, you may click on any of the following forms and print them out yourself.  Although we have online registration, we are not electronically connected to your healthcare provider.  These forms must be brought to your student's healthcare provider at his or her appointment time and returned or faxed to Franklin Middle School.  Be sure to sign or fill in the parent portion before submitting it to your student's healthcare provider to finish and fax to us.  Our fax # is 630-682-2340

Child Health Examination

The school code of Illinois requires that each student entering school for the first time (preschool, kindergarten, or first grades), students entering sixth and ninth grades, and transfer students must submit evidence of a physical examination done by a licensed physician, advanced practice nurse or examining physician assistant and a complete record of immunizations. 

Students who do not meet the health requirements will be excluded and will not be permitted to attend school per District Policy 7:100 Health,Eye and Dental Examinations; Immunizations and Exclusion of Students.  Unless an exemption or extension applies, the failure to comply with the health requirements by the first school day after labor day of the current school year will result in the student's exemption from school until the required health forms are presented to the District. New students who register after the first school day after labor day of the current school year shall have 30 days following registration to comply with the health examination and immunization regulations. 

Athletic Physical Exams 

A sports physical exam is required for students participating in interscholastic athletics and is valid for 395 days. The school health required physical examination form can be used for athletics, but a separate copy must be provided to the athletic office.
