Forms & Fees
Two forms are required for participation in interscholastic athletics:
1. A current physical (must be on file with the school to participate in all sports; necessary to try out for cut sports) (link to printable physical form)
*Physicals can be turned in to the Health Office or Athletic Director on paper or electronically.
*Midwest Express Clinic offers sports physicals for $30. Service at this clinic is an out of pocket cost and is not covered by insurance.

2. CUSD200 Athletic Participation Form ( coaches will distribute electronically; submission required to participate in any sport; completed after rosters are set for cut sports)
Interscholastic Athletic Fee:
The current athletic fee is $125.00 per student per interscholastic sport.
You must pay for your child's athletic fee on PushCoin after the fees have been posted to your child's account. It may take a couple weeks into the activity for the PushCoin account to be updated - please be patient. Coaches will be made aware once fees have been posted and will let parents know.
*There is no fee for participation in intramurals
