Below you will find the most current copy of the sports physical form that you will take to your physician. Please check that the physical has been signed and dated by the physician. You will then upload a copy of the physical and complete all other forms online when you register your student-athlete through the Wheaton Warrenville South Arbiter Registration . The Arbiter registration along with the physical upload online must be completed prior to tryouts.
