Transcript Requests

AN OFFICIAL HIGH SCHOOL TRANSCRIPT INCLUDES HIGH SCHOOL GRADES, GPA (GRADE POINT AVERAGE), AND CURRENT SCHEDULE (if applicable).

NOTE : Transcripts do not include standardized test scores.  To have test scores sent it is necessary to contact the appropriate testing center:

              SAT, PSAT – www.collegeboard.com, (866) 630-9305 
              ACT, PLAN – www.act.org, (319) 337-1270 

A $5.00 processing fee is assessed for each transcript. 

Transcript requests will be processed within 7 to 10 schooldays of receipt. Please allow this time frame in your deadline.

An official transcript cannot be released to an individual. It must be mailed directly to the appropriate agency (school, scholarship, employer, etc) by the Registrar’s Office.

CURRENT STUDENTS

In order to have a transcript sent, students must submit a Transcript Request Form to their counselor and add the school to "Colleges I'm applying to" in Naviance. 

A $5.00 processing fee is assessed for each transcript. A $5.00 charge will be added to myschoolbucks for each transcript sent. Students who qualify for free/reduced lunch or have a fee waiver will have that fee waived. 

As a part of the "Senior Survey", a “Final” transcript will be sent to the college that is identified. This is required by all colleges as proof of high school graduation. A student’s college 2nd semester course registration may be delayed if the college does not have the final transcript.

FORMER STUDENTS

Transcript and immunization record requests can be accepted through the mail (1993 Tiger Trail, Wheaton, Il 60189) or in person. Emails, phone, and fax requests are not accepted. 

A $5.00 processing fee is assessed for each transcript. The fee is to be paid prior to the release of the transcript (official or unofficial). Checks are to be made payable to CUSD200. Credit card payment is not accepted. The registrar does not have an ATM.  

If a student is 18 or older he/she must request the transcript.

A copy of a photo ID with a signature must accompany each transcript and immunization records request.
